Detail is the whole job

Healthcare fit-out is not a harder build than a shop or an office. It is a more detailed one. The same partition, ceiling and services work happens, but every junction has to be cleanable, every basin has to be the right pattern in the right place, and rooms have to be the size and shape the published guidance describes rather than the size that fits the plan. A treatment room that is fifty millimetres short, a coved skirting that stops at a doorway, or a hand-wash basin with an overflow are the details that fail an inspection after the money is spent.

We start with the regulator and the guidance, not the layout. Which activities are being carried out, whether the operator is registering with the Care Quality Commission, whether there is imaging, and whether decontamination happens on site all shape the plan before a wall is drawn. From there it is the same sequence as any commercial project: survey, drawings, landlord approval and licence to alter, building control, a fixed price, then the build. In London the constraints are usually the building: clinics regularly occupy converted retail units, upper floors and period buildings, so ventilation routes, drainage falls, floor loadings for imaging equipment and step-free access all have to be resolved rather than assumed. Where a practice is already trading, the work is phased room by room and the noisy stages fall outside clinic hours.

One crew matters here because the finishes are unforgiving. Welded vinyl, coved skirtings, sealed penetrations, silicone-free junctions at worktops and a level substrate under all of it depend on trades not undoing each other. Our site manager holds the sequence and the specification, so the fit-out you are inspected against is the one that was drawn. Where a detail is guidance rather than regulation, we say so and let you decide, rather than presenting a preference as a requirement.

    Lead-lined partitions and door sets, controlled area signage and interlocks, designed with a radiation protection adviser.

  • Ventilation, water and drainage

    Air change rates for clinical rooms, water systems designed against Legionella risk, and drainage with the falls the equipment needs.

  • Infection-control finishes

    Welded sheet flooring with coved skirtings, impervious wall finishes, sealed junctions and cleanable ceilings throughout.

  • Reception, waiting and joinery

    Reception desk with an accessible section, waiting seating, records storage and staff facilities.

  • Commissioning, certification and handover

    Activities, guidance and survey

    We establish what will be carried out in each room and which guidance applies, then survey the unit against it. Access, ventilation routes, drainage and floor loading are checked before any layout is drawn.

What governs a healthcare fit-out

    HTM and HBN documents set the expectations for ventilation, water safety, decontamination and room layouts. They are guidance rather than regulation for private practice, but they are the benchmark an inspector and a professional indemnity insurer will use.

  • Care Quality Commission registration

    Providers carrying out regulated activities must register with the CQC, and the premises form part of that assessment. Cleanability, infection control, accessibility and the suitability of each room are examined, so the fit-out has to support the application rather than follow it.

  • Ionising Radiations Regulations 2017

    Any room using X-ray equipment needs a radiation risk assessment, a radiation protection adviser, a defined controlled area and shielding designed rather than assumed. A critical examination is required before the equipment is used.

    Step-free approach, door widths, accessible WC provision and a reception counter with a lower section are all controlled. In converted retail units and upper floors, access is usually the constraint that decides whether a unit is viable at all.
